Added an initial VBScript (work in progress) that helps to create a fresh new Access database file (MDB) so we can eventually work towards removing the template database file. The script is more transparent and brings the AccessErrorLog solution on par with others where the database is also created programatically via scripts or at runtime.

* Added abstract class DatabaseErrorLog to simplify database implementations
* Added Error.ToXmlString to prevent code duplication
* Added Error.NewError factory methods to simplify code
* Organised usings in all database implementations
* Removed unnecessary processing in GetErrors when errorEntryList == null. Most implementations generated the error list entries even there was no list to add them to!
* Improved SQLiteErrorLog to take advantage of new code in DatabaseErrorLog which allows for Data Source=|DataDirectory|\Error.db in addition to ~/App_Data/Error.db

Fixed issue #63 (http://code.google.com/p/elmah/issues/detail?id=63). The hour component of "Time" column in the generated CSV should have been in the 24 hour format.
